Four points:

1. The distance from the extender outlet to the sensor needs to be very 
long so that no air reaches the sensor from the outlet.

2. The seal between the tail pipe and your extender must be 100% air 
tight.

3. The sensor must be mounted such that the body is exposed to fresh air 
at all times.

4. As you note, you would need a heated sensor.

I would imagine it was easier to just mount a standard unheated sensor 
permanently. One of the real cruxes of the O2 sensor is that it will give 
readings under all engine conditions, not just idle.
