If I detect body filler on a car, I will make sure I remove it all down to
bare metal.  The reason is that I don't know what that filler is hiding.
Was it done as a filler or to cover over a problem?  How thick is it?  Is
it a problem?

Ideally, you should remove all body filler and see what lay underneath.
You should then start metalworking to remove as much of the dent/damage as
possible.  Body filler should only be used for that small amount you can't
otherwise fix without replacing the panel.  Anything more than 1/4" deep
is asking for trouble and an indication you need to spend more time on the
metalwork to bring it closer to the original shape.  Surface weld stud
guns are ideal for this sort of work to help pull dents out.
