Doug,
Here in Western Washington (particularly the Seattle area) we have service split
between GTE and US West.  I can call as far away as West Seattle without it 
being
Long Distance.  But I have to pay LD rates for calls to Bellevue which is only 
15
or so miles away.  Other than that we pay no per minute fees for local calls.
