[Fot] exhaust PCV

This sounds probable. Perhaps more likely on some engines than others.

I tried to imagine the conditions that would create this issue with a wet sleeve engine. Didn’t come with anything.

It has been effective for us. It is reported to aid in Ring Sealing, too.
