I have an A-type OD but I think the shifting technique should be the same
for the D-type OD.

When I shift under power I just use the lever to engage the OD.  My A-type
really kicks in and it's a lot smoother when if I'm accelerating versus
coasting when that sucker engages.  If I want to shift to OD when I'm not
under power or just coasting I'll use the clutch.
