I'd say you should drain/flush/refill at least every two years.
If the coolant you flush out is discolored in any way, you should do an
extra flush cycle with a radiator cleaner, in accordance with the
instructions on the bottle.
The prime reason for reflushing every two years is that good antifreeze
contains very efficient corrosion inhibitors that eventually wear out.
