This clutch is to much.

Can anyone tell me the following:

1.  Which hole of the three in the clutch shaft does the slave cylinder 
piston attach to?  
2.  I seem to have a frothy substance to the hydraulic fluid surfacing at the 
top of my master clutch cylinder.  It is almost bubbly? Is this normal?

Would appreciate anyones thoughts....thanks Virgil 75 TR6
