Doug,
Your first mistake was buying from VB. Try The Roadster Factory, Moss or
British Parts Northwest before resorting to VB, because they sell a lot
of junk. Get TRF's fine catalogs and the Bentley and Haynes manuals. 
No, you shouldn't need an adjustable pushrod, just the right part
installed correctly. I've never had a slave cylinder suddenly just
completely stop functioning. Usually they start leaking and slowly loose
their ability to push fluid. Did all the fluid suddenly leak out through
the slave cylinder during your drive? 
The rebuilds don't usually last very long so I'd order a new one from
one of the reputable sources above as a back-up.

New tr 6 owner which Dick Taylor was the surrogate buyer. Developed leak
in clutch slave cylinder ( stranded on side of road) Ordered form
Victoria British LTD new slave cylinder Part # 7-589 and repair 
kit
Installed new slave cylinder today
Piston moves outside of housing thus leaking fluid.
Completely different set up than original slave.
Does anyone have experience with new slave Cylinders.
Do I need adjustable rod as shown in Roger Williams new book. Putting
new seals in original slave cylinder. 1971 car with 1972 motor
