I've finagled them off with a pair of vicegrips... er locking pliers.  Channel-lock... er slip-joint pliers work as well.   Grab it near the base and wiggle back and forth and it will ever so slowly work its way out.  The pliers will slip off several times but persistance is called for here. 

I've also seen caps with a hole drilled in the center where a screw can be screwed in and used to pull the cap off.  I've seen others where the screw can be used to push it out against the stub axle.
