[TR] Is there a rule of thumb on when to use a flat washer with a lock nut/nyloc?

I've not read Carroll Smith's book yet, but IMO one good reason for using a
flat washer is to spread the load whenever the underlying material is
softer/weaker than the nut.  Sheet metal or aluminum always gets a flat
washer.  'Split' lockwashers also tend to tear up castings, so use a flat
washer there, too.
