The tabbed washer should have a bend over tab on it to keep it in place.
OR the left hub has been replaced with a right hub.
The nut should be left hand thread so it does no "undo" when going forward.
If the hub was changed, or the left/right hubs switched at some time, 
this will cause your problem.
