It creates a vacuum on the bleeder screw.  But it also has a container 
that sits in the master cylinder that keeps it from emptying the MC.

This is a huge time-saver alone if you bleed brakes when the car is on a 
lift.  (I don't have to run up a ladder to check the MC.)
