[Healeys] Splines and Wheels

Figure this one?  I have had two Spline failures.  
1.  Right rear wheel spline got wiped out and I heard a very loud grinding sound when I applied the brakes.  I was able to drive home.  It was almost impossible to get the Knock Off Nut off.  I was eventually successful using a 20 LB sledge hammer.
2.  Left front hub spline got wiped out when I applied the brakes coming up to a stop light.  I heard the same grinding sound.  I knew that the spine was gone after the rear wheel incident.  I slowly drove into a parking lot. The Knock Off Nut was loose.  I called a fellow club member for assistance.  He brought me a floor jack and with the wheel off the ground we tightened the Knock Off Nut as tight as it would go.  I then proceeded to slowly follow him home using the hand brake to stop.  Unfortunately when coming up to the next traffic light a guy cut me off and I made the mistake of hitting the brake pedal.  Again the grinding sound and then the wheel came off, continued down the road into the side of a pick up truck.  Fortunately I was going slow enough that I didn't damage anything.  The car just dropped down on the Brake Rotor.  I retrieved the wheel and was able to get the car off the ground so as to put the wheel back on and proceeded to my friends garage where I left it until I could get it home and replace the Hub.  I have since replaced all Hubs, Knock off and Wheels.
Question why did the left front loosen and come off while the right rear tightened to a point that I was ready to torch the wheel off?
