I have a pair of doctor made custom fit earplugs.

They have interchangeable inserts that allow for different levels of  
dB reduction that is basically flat so that things still sound the  
same, just quieter. As a musician they are completely necessary. They  
are very comfortable as they aren't always trying to expand like foam.  
Great for driving.
