I've always had great success simply putting a 'sharp' edge
screw driver against the cap right at the edge of the hub...
Angle the screw driver.. handle near the drum...tip pointing
in the direction the cap needs to move.
hit it once or twice with a hammer (the screwdriver).
It will move the cap out just a little bit.... then rotate and work your
way around the cap.
After removing the cap for the first time...I drill a hole and insert
a small (#6 - #8) machine bolt through the hole (head inside the cap)
install a nut ( or two) on the outside of the cap. Now you have a handle
to easily grab with a pair of pliers to remove it next time.

The method recommended in the manuals is to drill a small hole in the centre of
the cap (if one doesn't already exist). Then screw a screw into the hole.
When the screw reaches the end of the stub axle, keep screwing and the thread
will pull the cap out.
Of course, what will probably happen is that the cap will be so firmly fixed
that the thread will simply enlarge the hole until the screw can rotate
freely. At this stage you smash the cap to pieces with whatever tools you
have available, and order a new one.
One suggestion I have heard is an extension of the screw approach. This
involves welding on a small nut just inside the cap, before replacing it.
Then you actually have a decent quality thread, so screwing a bolt in will be
much more likely to work. This also has the advantage that you can put a very
short cut-off bolt into the thread for the rest of the time, to prevent any
muck getting through the hole.
