Try a new fuse anyway.  I've seen the flat type fuses get hairline cracks in
the part that's supposed to blow, and tube type fuses where the wire came
loose at one end.  Both situations caused intermittent operation of the
protected system.

About 60% of the time the heater/airconditioning fan works just fine.  All
three speeds.  About 40% of the time the heater/airconditioning fan does not
work at all. There seems to be no pattern to this.  Obviously, the fuse is
not blown, since it works sometimes.
