Sounds like there must be something wrong inside the handle : My guess would be the pawl pivot has fallen out (or been removed and replaced improperly). Could be the ratchet is broken or missing too

Also FWIW, I do it much the same way, except I fit both shoes into the cylinder first, then both ends into the adjuster. Probably makes no difference on your TR4A with it's single-acting cylinders,

The danger for either location is that the associated shaft will somehow move while you're adding oil. Fairly unlikely I would think, provided you don't turn the drivetrain at all. BTW, I was wrong
