Hi Jim, Take a good look at the U-Joints on the drive shaft...my guess is the rear. It is possible the drive shaft is out of balance as well...threw a weight? It could also be the the UJs on the axle

I am inclined to agree with you. The issue seems to be one of two things in my mind, tire balance or front wheel bearings. It doesn't happen in third, because the tires are not rotating fast enough.
Do you happen to notice what speed this vibration occurs? Meaning, as soon as you put it in 4th it starts vibrating or is it at a certain speed? In theory you could go into 4th at a low speed & test?

Jim--The usual suspects are any of the U-joints, but since you've had this vibration for some time, it should've gotten worse. I'll suggest that you check out the exhaust system, front to back. Bang
