But - I swear - I've never touched the distributor. The closest I've been

pulling out the tach gear. That's it. I've not loosened the clamp bolt or 
anything else (that I remember) and still the rotor/spindle/cam is turned 
180.

Have you owned the car since new?  Did you replace the spark plug wires? 
Is it possible that a previous owner had the distributor out and put it
back in 180 out and compensated by swapping the spark plug wires around?

Or is the rotor defective?  Compare it to the other rotor.  Maybe the mold
was put together wrong and there is a bunch of them out there like that.

It is highly unlikely that a timing chain or a distributor drive gear
skipping a tooth would wind up exactly 180 out.  I'd rule that out until
after EVERYTHING else checks out.
