SPIT FUEL PUMP

The fuel pump failed on mu spitfire yesteday.  So much for crusing around
and enjoy 80 deg. weather in October, in Ohio.  Back to the fuel pump it was
not an old pump and the failure mode has me concerned.  The lever arm broke
off and then fell inside the engine.  A couple of questions 

1) Could it have been installed wrong?  
2) Whem I remove the oil pan, should I be able to remove all the debris, or
do I have tear more deeply into the engine. 
3) What is the deal with the different style of fuel pumps, VB recomends
 a bent arm and then getting an spacer.


 For those who wish to read on, Gee maybe this is why I have been having
carb (I though) problems all summer.  Also before I got into the fuel pump I
removed the gas tank and flushed it out. I did get a good bit of rust flakes
out, so it was not a wasted effort.
