Clutch Slave Cylinder Question

1) I am rebuilding the clutch slave cylinder and my old Autopress manual 
indicates that the bolts are removed from the flange on the bell housing 
and then the cylinder is removed by pulling the cylinder toward the front 
of the car leaving the push rod attached to the clutch actuator arm. 
Problem is that mounting holes are behind the bell housing mount and the 
cylinder can not be removed without disconnecting the actuator arm and 
removing the cylinder toward the rear. My question is: has some PO replaced 
the proper arm with a short one necessitating this change from proper 
design or was there two ways it was done originally and this is simply a 
variant not allowed for in the manual I am looking at?

2) Being a relative newbie to the list, just what does LBC stand for?
