All,
    I have a 3 point where the inertia reel unit mounts on the real
wheel arch near the floor.The belt then goes straight up to a pivot
point on the wheel arch about shoulder height so there is no discomfort.

Bob,

My '74 spitfire has 3point belts.  I shoulder strap is attached to the
floor behind the seat and come over the shoulder part of the seat.  When
the belt locks, it is not fun cause it puts tension downward on the
shoulder :-)
