Boring a block to accept pistons is no easier cheaper or quicker then boring a 
block to accept sleeves.  Dropping a sleeve in is quite easy to do.  If one is 
nervous about doing it themselves, have the machine shop do it when they finish 
up the bore job.  It'll add about 20 minutes time to the whole job.

My guess would be because it's usually cheaper to find another block that
just needs boring than to sleeve even a single cylinder.
