I had the same symptoms and they were correctly diagnosed by Dave Porter.
The problem was a failed sealing o-ring on the outer diameter of the
accumulator piston liner. The accumulator works as a pressure controller and
fluid reservoir by receiving the oil output from the pump and building
pressure against the springs behind the piston. If the oil leaks between
the outer housing and the overdrive housing you won't be able to build
enough pressure to make the OD work reliably. The o-ring is cheap from the
usual suspects, the repair is a little more challenging, but it's worth it.
