[TR] Is there a rule of thumb on when to use a flat washer with a lock nut/nyloc?

Don't know if there's a "Triumph" rule, but Carroll Smith (wrote all 
those "Tune to Win" books) wrote that you should always use a flat 
washer under a nut.
