Shoulda had Frank make me one!  I just might have to one myself.

I got extra spacers too.  There is only one/side.  I had a clunk in one of
my springs(15 leaf type) turned out one of the clamps was loose.  I don't
think this applies to your case but the bolt holes in the mounting plate
can enlarge.
