Did a little searching (with help from my List friends).  Here's what I found out:

Dale Mfg - recommended a while ago; appears to no longer be in business (at least, the domain is up for grabs)

Damper Doctor - approx. $330 to rebuild + shipping + tax

Damper Dudes - approx. $130 + shipping + tax

Take a WAG which one I'm going to send my damper to.
