HMSA has web site, and you can get application and medical forms, but
not event application forms.  You need the Acrobat reader, but you can
download it from their site.  The URL is:

Just move your pointer over the URL, and when the arrow changes into a
hand with index finger, double click. It will take you right ot the
site.
