Near the bottom of that page, you should see a button labeled "unsubscribe
or edit options". Click it to get the unsubscribe page. Fill in your email
address & password, then click 'Unsubscribe'. That will send you an email
message with further instructions (which amount to simply clicking 'Reply'
and 'Send' in your email program).
If you don't remember your password, you'll need to first click on the
button at the very bottom of the page, which will send you an email with a
password reminder.
