The bolts go in through the top of the rail into the floor.  Some of mine have 
nuts welded to the underside of the floor pan -- which I suspect is authentic 
-- and some have loose nuts.

One of my loose nuts is directly above the frame rail, and there is a hole in 
the rail under it.  I used a 7/16 deep socket through the hole and the nut 
dropped into the socket.  No problem.

Getting it back together was a *little* trickier...
