The latter is correct. Press out the old, press in the new, then ream.

GAAAA! (General expression of panic.) DON'T USE AN ADJUSTABLE REAMER!
The two bushings have to be reamed together and perfectly in line with each
other. To use an adjustable reamer, you would have set the size, to ream
one bushing, then adjust, then ream the other, and somehow have both
reamings come out perfectly concentric. Good luck. There is a proper
(non-adjustable) tool that reams both bushings in one pass, and gets them
concentric. Borrow one of those, or have a local shop that has the proper
tool do it.

Don't make the mistake I made. trunnions go on the kingpins with he part
that takes the shock arm TOWARDS the brake disc. The car looks pretty
darned funny the other way. (i.e. \ / is the front view)
