Mike, I also found this linke to McMaster-Carr that shows the "shakeproof" 
washer a "tooth lock" washer and they show just about anything you want to know 
about washers.

Mike, Google "star washer" and you will get all kinds of examples of a 
"shakeproof" washer.  I've always called them star washers.  Hope this helps.
