They don't normally fall out on their own.  I'd suspect you bent or
broke something.  The lash pad should be on top of the valve inside the
retainer.  
The flat side goes down and the rocker arm fits into the channel on top
of the lash pad.  If you put it all together and you have excess valve
clearance you probably dropped that valve seat.  Check that neither
valve spring is broken also.  You use a valve spring compressor to push
the valve open while you insert the rocker arm.  Unfortunately the tool
rental places don't usually have a spring compressor that'll work well
on the U20 head.  You can screw the pivot (the part you turn to adjust
the valve clearance) all the way down and usually get the rocker arm in
place but then you can't check how much clearance you've got now - with
the problem.  You could also put air pressure into the cylinder through
the spark plug hole to see if the valve in question seals.  If not, it's
bent or the seat fell.
