Thanks to all for the help on this one... 

Turns out that in my case it was a little of basically everything anyone
suggested...  The bushings in the firewall were missing, as well, the
linkage was assembled wrong!  Oh well... it's back together now and so
far (knock on wood) seems to be working as expected!

That said, now that I have had it out for a bit, I have noticed that on
occasion that gas pedal seems to feel like it is binding.  When I press
on it, I get up to about 2500rpm and then the peddle feels like it has
hit a stop.  If I remove my foot and re-engage the pedal I almost always
get full travel again and everything seems back to normal.
