My first guess - and it is one - is that the blocks that were used were not structural concrete blocks, but the type used for block walls. What is the soil like? Heavy Alkaline? I've never seen this

Wow! I've never seen anything like that! Where are you located, and do you know what the required foundation depth is?! It's based on the freeze depth in the region. Back in Ohio, I think it was 6ft;
