The upper A-frame bushing on the passenger side has a lot of play. The driver's side doesn't feel bad; should I do them both? I understand the lower ball joints also usually are bad at the same time.

The upper bushings are screwed in and you need an impact gun to remove the old ones. Breaker bars just don't cut it and even some impact guns just don't do it either. Not hard to do but have a big v
