IMO if it's a bolt and nut, then generally no need for a flat washer under
the head, since it doesn't turn or take a lock washer. But if the bolt
threads into a casting, then generally a flat washer is appropriate.
Generally, though, I go with the original arrangement as shown in the parts
catalogue :^)
