Are you pushing the plunger all the way down when bleeding?  The front and 
rear pistons are not mechanically connected.  The column of fluid from the 
firewall side piston is what pushes the front piston forward.

If the firewall side piston is pushed a short distance, it will never push 
the front piston during bleeding.

If that isn't the case, the piston could have gotten stuck at the end of the 
bore or could have a torn seal.  If it has a torn seal, some fluid should 
still flow.
