Yes it is "not uncommon" for third to pop out of gear. The likehood is that
the springs in the hubs. item #25 on page 93 VB catalogue TC 19.
It requires a dismantle of the Xmission.
Alas, you know I'm right when, you have to spend $500 to change a $0.35
part. (only there are 3 per hub and two hubs.) I change them whether they
are good or bad whenever I tackle a Xmission. most likely whomever did the
rebuild nickeled you to the tune of a rebuild.
As for the shift cover being on too tight... I think whomever told you that
had his/her shifter on too tight.
Reminds me of the definition of indecent:
If it's in far enough...
If it's in hard enough..
If it's in long enough...
it's indecent.
