Er, have you tried all those tests for fuel coming out with the gas cap off? If not, it could be the tank's not venting. Pull the fuel line off the tank and check for fuel running out there. Blow air at low pressure into the tank outlet if not. Blow air back thru fuel line from the tranny end to clear it out.
