Are you also tightening the truncated-inverted-cone-shaped chrome piece just
below
the knob?
Here is how I replace the knob: screw the chrome piece down as far as it will
go.
Then put on the knob, and screw the threaded ring just until its top surface is
flush
with the top of the shift lever. Then screw the chrome piece up until it is
tight
against the knob, with a wrench. You never have to apply any
real torque to the little threaded ring.
My knob has never gotten loose.
