He doesn't say how many miles/km are on the car, but it could be that
the flywheel face has worn down to where it is not thick enough to
properly mate. He also doesn't say if the driven disc and the pressure
plate are of the same manufacture, but that can make a difference.

BTW, I assume that the 'small tube that comes out of the bell housing
that the clutch release arm slides on' is the release bearing carrier.
If not, could he be talking about the pivot pin?
