Patrick,
Yes, you can update an existing tranny with an Overdrive, but as you
mentioned the output shaft is different, so it has to be replaced with
one specifically manufactured to support the overdrive unit.

P.S.  It's much easier and probably cheaper to simply buy an Overdrive
Tranny, though!
