I find that tapering the end of the shaft just a little bit, along with
expanding the u-joint helps immensely

Taper the shaft with a grinder, only one to two mm on the end, with a length
of no more than three mm.  This has no affect on the holding power as all the
ground parts will not be in the clamped area.

I use a chisel, insert it in the u-joint, and twist to open it up.
