BMIHT Certificate

Has anyone recently requested a Certificate??? I mailed for one back in early 
Nov and still nothing. I suspect my order has been lost because my check has 
not cleared either. Is there an "Email" Address to contact their office??? 
Any help would be appreciated. THX
