Jason:
I have never heard of one slipping out. But I have heard many tales
and have one personal instance of the gear teeth stripping and the car
coming down hard. Good used MGB jacks for a long time were very hard to
find because of this problem. Inspect the jack very carefully and never put
yourself in danger when lifting the car with one.
