Dave- My guess would be a blown head gasket (between 2 & 3) I would look for an oil slick at the radiator filler or a mayonnaise type stuff on the oil filler cap. Sometimes neither of these conditions will exist and the compression leakage will go back and forth between cylinders. The head needs to come off to be sure.
