Shame on me! (MG '96)

I was planning to sign up for MG '96 a few (well, OK, several) months 
ago.  Since then, things have been insane, and I A. forgot all about it, 
and B. accidentally erased all pertinent info.  Could someone please send 
me any useful info, particularly dates and contact phone #'s?  I'm not 
sure it needs to be rehashed on the list again.
