John,

I do not think you are fumble fingered.  I opened the file into Acrobat and
found the same problem.  I am not familiar with Acrobat, not sure if there
is a way to edit a document when opened.

For now use the tried and true way that even us technophiles use at times:
Open the document, print it, use a pencil to inscribe all of the information
(including Visa number) and then snail mail it.  If you want to Email just
type in the info it is asking for (including Visa) and Email to:
office@scta-bni.org

Sorry that is the best I can do for now, the wife (read computer instructor)
is out tonight.  I will ask though.  I promise this will be better next
year.
