Sure we can blame Outlook -- but an Outlook on the machine of someone else
who happens to have you in his address book.

That's the likely scenario because that's how Klez and Bugbear work.
Unlikely that your ISP got hacked. Nobody specifically targeted you or your
address -- the viruses steal them from address books at random. If the
victim has president@whitehouse.gov in his address book, you could get one
that purports to come from Dubya hisself.

Man, that was a LOOOOng Thanksgiving! Bet there's no leftover turkey! :-)
